天冷吃鍋補過頭 小心蕁麻疹來敲門

冷氣團來襲，與朋友相約大啖各式鍋物，尤其是冬天好朋友：麻辣鍋、麻油雞、薑母鴨…，不吃太可惜~但你知道嗎？有冬季乾癢問題者，若時常攝取溫補鍋物，或泡湯、烤三溫暖，可能使皮膚搔癢加劇。
亞洲大學附屬醫院過敏免疫風濕科主任蔡肇基表示，近期寒流來襲，深受皮膚乾癢就診的患者明顯增加2成，均是「蕁麻疹」惹得禍。
患者主訴因吃鍋、泡湯、烤三溫暖後，全身皮膚反覆出現紅、腫、癢的膨疹，雖隨著身體機能代謝，症狀慢慢緩解，但發作期間卻因搔癢，常抓到皮膚破皮流血，嚴重影響生活品質。
冬季是蕁麻疹好發時節，原因是空氣濕度降低，肌膚無法及時調節適應，加上早晚溫差大，皮膚黏膜水分被快速蒸散，變得極度敏感，若遇到促發因子，很容易就誘發蕁麻疹。
蔡肇基說，當過敏原引發皮膚或血液中巨大細胞的活化反應，使之釋出組織胺等發炎釋放物質，皮膚就會出現血管擴張的紅色膨疹，誘發原因包括氣溫急遽變化、食物、藥物、心情低落、工作壓力大、熬夜等。
多數蕁麻疹可於1個月內治癒，但若症狀超過6週，則成為慢性蕁麻疹，需進一步找出原因。蕁麻疹用藥為抗組織胺，以弱化巨大細胞對身體的影響，若是患者抓癢抓到破皮，則會另外使用消炎藥，或皮膚用抗生素。
蔡肇基指出，除了蕁麻疹，冬季皮膚過敏原因多，包括異位性皮膚炎、慢性濕疹、自體免疫疾病(如全身性紅斑狼瘡、乾燥症、甲狀腺疾病等)，都會以紅腫、乾癢、乾裂等症狀表現，定要赴醫檢查對症下藥，才能擺脫惱人冬季癢。
他提醒，不論任何疾病造成的冬季癢，都可透過塗抹乳液、護手霜為肌膚保濕；如角質層抓破，則可合併使用凡士林、止癢藥膏及類固醇藥膏；若有出血的傷口，清潔後可用含銀分子或抗生素的藥膏治療；最後，因鹼性肥皂會抑制皮膚保護蛋白，而造成乾裂，故平時可用弱酸性(pH 6)肥皂清潔身體較佳。
